Hoscheit RN Nurse Hours: School days from 7:30 AM to 2:30 PM Athletic Trainer: Sarah Anderson, ATC Trainer Hours: The Athletic Training Room will be open from 2:30 PM to 5:30 PM on regular school days. There will be coverage for every home event. The Athletic Trainer will travel with Sophomore and Varsity Football. On non-school days, the Athletic Training Room will be open based on practice/game schedules. A current health exam (dated less than one year prior to the start of school) is required for all 9 th grade students. This must be on file in the Wellness Office by August 1. All immunizations must be up to date. The physician or health care provider must sign and date the form verifying that immunizations were administered. The Health History section must be completed and signed by the parent. The examining physician/nurse practitioner must sign and date the form. The office address and phone number must be included. If the student is required to have a sports physical in the year that coincides with this exam, then this also will be accepted as an IHSA sport physical if the statement regarding participation in interscholastic sports is completed by the examiner. Only the current (R-01-05) physical exam form will be accepted. Please keep a copy for your records!!! (Download the Certificate of Child Health Examination) The 9 th grade health exam must include all required immunizations. At a minimum this includes: 1 current Td or Tdap vaccine (within the past 10 years) Proper documentation: month, day, and year for each dose administered is required. Medical Exemption: a statement from a physician indicating that the immunization in question is medically contraindicated. A current medical objection letter must be on file in the Wellness Office by August 1 st or within 30 days of enrollment for transfer-in students. Religious Exemption: a letter dated and signed by the parents detailing their religious belief which forbids the specific (name the vaccine\s) immunization. Standardized forms are not acceptable. All letters will be reviewed by the local school authority. A current religious objection letter must be on file in the Wellness Office by August 1 st or within 30 days of enrollment for transfer-in students. Out of State\Country Transfer Students: at ANY gradelevel, must present a health examination dated less than one year prior to the enrolling date on a comparable form. In State Transfer Student: A completed9 th grade health exam on the current form (R-01-05), with all immunizations up to date. If the physical does not meet the above requirements the student will be given a 30 day grace period to comply. It is the policy of St. Francis High School that the administration of medication, or supervision of self-medication to students during regular school hours should be discouraged unless necessary to maintain the student in school or in the event of an emergency. No school personnel shall administer to any student, nor shall any student possess or consume any prescription or non-prescription (over the counter) medication unless the Authorization for Administration of Medication form is on file in the Wellness Office. This form may be downloaded from the St. Francis web site and must be signed by the parent and completed by a physician prior to dispensing of any medication to the student. All medications must be delivered to the school nurse (or in case of the nurse’s absence a designated administrator) as soon as the student enters the building and prior to class. A student may keep prescribed medication in his or her possession only when a physician has verified that a student must possess the medication for emergency reasons. The school nurse (or in case of the nurse’s absence, a designated administrator) will administer the medication or supervise self-medication in accordance with the physician instructions. Please note: medication is to be in the original, labeled container as dispensed by the pharmacy or the manufacturer. It must state the student’s name, name of the medication, directions for use and date. ATHLETES WILL NOT BE ALLOWED TO PARTICIPATE AFTER BEING SEEN BY A PHYSCIAN WITHOUT A NOTE!!!
St. Francis will follow the Center for Disease Control guidelines for head injuries/ concussions. These guidelines will be discussed with the athlete, parents, and coaches per incident. Athletes must get taped before practice time. There will be no taping “just for games.” If the athlete does not get taped everyday, they will not be taped for game only, unless an injury has occurred during the game. There is also a no Spatting (taping on the outside of the shoes) for any sport. Hours: The Athletic Training Room will be open from 2:30 p.m. to 5:30 p.m. on regular school days.
